Kyler Care Connect Program (KCCP)
Kyler Care Connect Program is a dedicated case management program for individuals living with type 1 diabetes (T1D). It is designed to walk alongside you every step of the way. Our experienced case managers offer personalized support to help you navigate Insurance and coverage options, access to needed medications and TD1 technology, disability applications and social needs navigation for long-term sustainable support.
Is the Kyler Cares technology grant program still available?
The Kyler Cares Technology grant program is now part of the comprehensive Kyler Care Connect Program. If you need help with accessing, or paying for, needed T1D technology, your KCCP case manager will work to fill this need through your insurance coverage and/or applying for assistance through manufacturer or charitable patient assistance programs. If your case manager is unable to resolve your technology needs through these resources, they will evaluate your eligibility and, when appropriate, facilitate an application to the Kyler Cares Technology grant. This process will enable us to extend the impact of our limited grant funding by ensuring it is used only when your case manager cannot find support through other resources.
